Abstract

Introduction: This article explores the historical dynamics of the doctor-patient relationship, highlighting the importance of dialogue and active listening since ancient civilizations, emphasizing its importance in the context of clinical research. The concept of the “mental model” is introduced, emphasizing how individual physician and patient interpretations influence the interaction. In addition, current challenges such as information overload, defensive medicine, and barriers to access to care are raised, as well as changes arising from the COVID-19 pandemic and the influence of technology. The relevance of codes of ethics to prevent medical malpractice is highlighted.

Contents: The four components of the mental model are described: biology, personal history, culture and unconscious expression, emphasizing the need for a relationship based on empathy and respect. The bio-medical and bio-psycho-social models and the importance of integrating both approaches are discussed. The impact of technology and globalization on medical practice, including telemedicine and precision medicine, is examined. The role of informed consent and the active participation of the patient in decision making, including his or her participation in clinical research, is discussed. The importance of extending the concept to a relationship between the consultant and the health team is raised, and the problem of the lack of medical personnel and the precariousness of the profession is addressed.

Conclusions: The article concludes that the patient-physician relationship is in constant evolution, influenced by historical, technological and social factors. The need to balance technological advances with patient-centered care and empathy is emphasized. The importance of addressing inequities in access to care and valuing health professionals is emphasized. The need for a strong and efficient relationship to achieve desired outcomes for both parties is raised. Finally, it reflects on the importance of humanity in medical practice, quoting the words of Dr. René Favaloro.
