Abstract

Background: Kidney transplantation is a vital treatment for end-stage renal disease, but graft rejection remains a considerable risk. Recent research shows gender differences in kidney transplant outcomes, such as rejection rates and risk of death.

Purpose: This study investigates how patient gender influences graft rejection, focusing on the Dominican Republic, where differences in rejection patterns are observed.

Methodology: A retrospective cohort study was conducted with 103 transplant biopsies between 2005 and July 2023. Data from the National Renal Biopsy Registry were analysed, with a focus on sex differences in rejection time. The time from transplantation to diagnosis of chronic rejection was measured.

Results: Of the sample (n=99), men (n=68) were more likely to reject than women (n=31). Women showed later rejection than men (median of 1,460 vs. 365 days, respectively).

Conclusion: Differences in renal transplant rejection time between men and women in the Dominican Republic could be influenced by biological, pharmacodynamic, pharmacokinetic, and socioeconomic factors, underscoring the importance of personalised approaches in post-transplant management.
