Abstract

Background: End-stage renal disease is a major public health problem. Achieving and maintaining long-term vascular access becomes difficult and challenging in those patients on hemodialysis for renal replacement therapy. The HeRO® graft device (Merit Medical), has emerged as a possible solution for patients with limited options for hemodialysis.

Purpose: In this article, a review of the generalities, indications, cost-effectiveness, disadvantages and potential complications of said device is carried out through a search of the literature available to date.

Methodology: A systematic literature search of published studies without language limitation was performed in order to obtain greater sensitivity and yield higher search results, using the electronic databases of MEDLINE, LILACS and Europe PMC until August 25, 2022. The words used for the search were "Renal Insufficiency, Chronic ", "Dialysis", "Renal Dialysis ", "Hero Graft access Device" and "Hero graft".

Results: The use of the HeRO® device has a promising future as it offers a novel way to avoid the deadly risks of prolonged catheter dependence. Careful patient selection, experience and creativity in endovascular surgery and interventionism, as well as experienced management by dialysis providers, will be required to promote and maintain quality outcomes.

Conclusions: The long-term quality outcomes of the HeRO® graft will ultimately define its place in the vascular access arena. The device offers a novel way to avoid the deadly risks of prolonged catheter dependence.
